Premature ovarian failure, also known as primary ovarian insufficiency, is a condition that affects woman under the age of 40 years old and is characterized by a loss of normal ovarian function earlier than expected. Although it is often confused with early menopause, they are not exactly the same. What is premature ovarian failure?
Premature ovarian failure occurs when the ovaries stop functioning normally before the age of 40. This involves a decrease in hormone production, especially estrogen, and a disruption in ovulation.
Unlike menopause, where ovarian activity ceases completely, in premature ovarian failure there may be intermittent activity. This means that some women may ovulate occasionally, which even allows for the possibility of spontaneous pregnancy in rare cases.

How is it detected?
Diagnosis is usually made through a combination of medical history, symptoms and hormonal tests. The most common include:

- Evaluation of ovarian reserve (anti-müllerian hormone or AMH).
- Gynecological ultrasound to observe the ovaries.
It is generally confirmed when FSH levels are persistently elevated in a young woman with absent or irregular menstruation.
Does it affect fertility?
One of the most concerning aspects is its impact on fertility. It's important to note that the probability of spontaneous pregnancy is not zero due to intermittent ovarian activity.
However, in the vast majority of cases, reproductive options involve assisted reproduction treatments. The choice of technique and treatment will depend on factors such as age, ovarian condition and AMH levels.
Detecting this condition early can make a significant difference. It allows not only for the initiation of appropriate treatment to improve quality of life, but also for exploring reproductive options at the most suitable time.
