The semen sample, whether from the couple or a donor, is key to achieving pregnancy. The higher the quality, the more feasible fertilization will be. But today, thanks to technical advances, results can be obtained even in cases with the worst prospects.

It is a process known as capacitación seminal: el proceso que permite a los espermatozoides adquirir la capacidad de fecundar el óvulo. Son unas modificaciones en su estructura y movilidad para que puedan penetrar la membrana que recubre el ovocito y fusionarse con él. Dependiendo del diagnóstico del seminograma, se realizará alguna de estas técnicas de capacitación específicas:
- Density gradient. This is the conventional technique and consists of using two media of different densities to imitate the phases of the natural female sperm journey. Only the motile and best morphological sperm will reach the last phase of density.
- Swim-up. Suitable for samples with poor mobility or concentration. Sperm are selected based on their ability to rise from the bottom of a laboratory tube to the top in the culture medium.
- MACS (Magnetic Activated Cell Sorting).Esta técnica elimina los espermatozoides muertos o los que están en proceso de morir, quedando únicamente los vivos. Es adecuada para pacientes con necrozoospermia. Con ella se aumenta aún más la probabilidad de lograr un embrión que dé lugar al embarazo.
- Fertile-Chip. In patients with repeated miscarriages, previous treatment failure, embryonic blockage after the third day, or advanced age (men over 47 years of age), a sperm DNA fragmentation test is usually recommended. If this test gives a positive result, the Fertile-Chip technique will be applied. This is a recent method for eliminating sperm with this type of fragmentation. It improves the success rate.
